  WAC 480-116-031 (5) provides that the annual report forms for all classes of 
water utilities shall be published by this Commission and any change will only 
be accomplished after due notice and order of this Commission.


  On January 12, 1996, all regulated water companies and interested parties 
were provided notice of proposed changes to the water annual report form.  
Comments were received and considered. The Commission finds the attached water 
annual report form appropriate, pursuant to the rule.


  IT IS ORDERED That the attached water annual report form is approved as 
required under the provision of WAC 480-110-031.


  DATED at Olympia, Washington, and effective this 
day of February, 1996.
