WAC 480-110-275 (3) provides that the annual report forms for all classes of 
water utilities shall be published by this commission and any change will only 
be accomplished after notice and order of this Commission.

  The Commission finds the proposed water annual report form appropriate, 
pursuant to the rule. 

  This matter was brought before the Commission at its regularly scheduled open 
meeting on March 28, 2001.  The Commissioners, having been fully advised in the 
matter, and having determined the following order to be consistent with the 
public interest, directed the Secretary to enter this order and related 
provisions.

O R D E R

THE COMMISSION THEREFORE ORDERS That the proposed annual report form, in this 
docket UW-010329, shall be and the same are hereby approved in this docket 
dated March 8, 2001.

DATED at Olympia, Washington, and effective this 28th day of 
March 2001.
